Illness or Injury at School
When a child is injured or becomes sick during the school day, a parent will be contacted immediately. If a parent cannot be contacted, then the emergency number listed on the yellow card will be called.
Please remember to give the school any new home phone numbers or new job phone numbers throughout the school year. This will enable us to reach family members in times of emergencies.
Immunizations
A child must have the following immunizations before entering school:

Diphtheria, Tetanus, Pertussis (DTP/DTAP)

Polio Vaccine (OPV/IPV)

Hepatitis B

Measles, Mumps, Rubella (MMR)

Varicella (chickenpox)
